2011-2012 Confirmation Calendar
Program Requirements (includes due dates for all requirements). 

The following forms are provided during the Candidate-Parent meeting or during class.

Confirmation Information Sheet

• A copy of your baptismal certificate needs to be attached to this form if you were not baptized at St. John the Baptist.
• If you cannot find your baptismal certificate, you can contact the parish you were baptized at and ask for a copy.

Confirmation Service Proposal Sheet

• The Coordinator of Confirmation is the only person that can approve your service project proposal.
• This form needs to be turned in 1-2 weeks prior to your date of service for approval. All proposals are due by Sunday, November 13 class session 3.

Confirmation Worship Notes

• Five Worship Notes are required to be filled out completely and thoroughly during and/or after Mass.
• You may attend Mass elsewhere if need be. Check the Confirmation Calendar for due dates.

Parent/Sponsor Volunteer Form

“Just a little about me…” sheet

• This form needs to be completed and given to your small group catechist (volunteer teacher).
• This form is a helpful way for your catechist to get to know you and to find out where you’re at

Attention: Make-up Session Work!

The following make-up session work is to be turned in due to a missed class session. Work should be submitted completely and legibly.  If not, candidates will be asked to redo their work.
Candidates are allowed to miss 1 class session due to serious circumstances,
such as illness or a death.
